The Visitation handbook outlines all of the school's rules and regulations for the current school year.

School Supply Lists
School supply lists are available by June 1st for the upcoming school year. Lists are available for each grade level, excluding preschool and kindergarten. See right for downloading the form(s) you need.

Acceptable Use of Internet
Since Visitation uses technology and the Internet across all K-12 platforms, students and parents are required to read the Acceptable Use of Internet
in its entirety and sign the Acceptable Use of Internet Signature Form before any hands-on work with technology begins.

Medical Form
Illinois State Law requires a complete physical for students entering preschool, kindergarten, 6th and 9th grades, or anyone transferring to Visitation from another state. Included with this physical must be an immunization record for measles, mumps, rubella, tetanus, diphtheria, poliomyelitis, and pertussis.

Dental Form
Per Illinois State Law, dental check-ups are required for students entering kindergarten, 2nd and 6th grades.

Eye Exam Form
Eye exams are required by Illinois State Law for all students entering Kindergarten.

Internet Safety Brochure
Published by Joliet Diocese, this brochure outlines the dangers of internet use for all ages, as well as tips and guidelines for supervising your child(ren) when they are on-line. Social networking is also introduced, as well as links to various informational sites and filtering software sites.

Videotaping Agreement Form / Opt Out Form
This is a required document for all families wishing to approve their child(s) image for use in school videotaping, website, and photography. Those families who do not wish their child(ren) to be photographed or videotaped, must sign the Opt Out of Pic/Video form.

Standards of Behavior for Working With Minors
Published by Joliet Diocese, this document outlines the behavior that is expected of all those individuals who are working with any child under the age of 18. Any questions regarding these guidelines should be addressed to the diocese.

Safe Environment Policy
This document outlines the necessary documents and forms to be signed by any adult who is working with minors in a classroom, group, or individual setting on school grounds.

Pastoral Policy on Abuse of Minors
This document outlines the diocese's no-tolerance policy on abuse of minors by a priest or deacon.
